The final cover art is of a pivotal scene from the book, featuring the main characters in the backdrop of the beautiful Neuse River. Penny Rooney, the 11-year old protagonist with Cystic Fibrosis, was drawn wearing a t-shirt with a purple rose to symbolise Cystic Fibrosis awareness. She also has a PICC line in her arm for IV antibiotics. The hazy river path in the background, leading up to the focus on the characters, symbolises the theme of distance in the book. The realistic but slightly whimsical colour palette was chosen to reflect the serious nature as well the hopeful, uplifting tones in the book.
